Listen here, buttercup. A quadriplegic is someone who can't move or feel much of their body, usually 'cause of an accident or something. The joke is that they're "unsympathetic" because they supposedly only feel a tiny bit—10%—of anything, get it? It's dark humor because it makes light of a serious disability.
