"Why are you not saluting like the others?" Hitler barks.
"Mein Führer, I'm the nurse," she responds. "I'm not crazy!"
The joke plays on the idea that giving a Hitler salute is a sign of madness or at least blind adherence, and it's ironic that the only sane person in the asylum isn't saluting. The humor comes from the unexpected twist of the nurse's sanity contrasting the inmates' behavior.
